Staff Pharmacist - Per Diem - CPS Solutions - Phoenix, AZ - work from home job Company: CPS Solutions Job description: Staff Pharmacist - Per Diem Haven Behavioral Hospital of Phoenix Phoenix, AZ As a per diem, nonexempt CPS Solutions, LLC Staff Pharmacist at Haven Behavioral Hospital of Phoenix, you will fill orders with accuracy, check technician-prepared products before release, maintain security of pharmacy and controlled substances, enter orders and patient information with accuracy, aid in the training of new staff, update the Director about the daily activities of the pharmacy, and aid the Director in maintaining the pharmacy. Schedule: This per diem, nonexempt position provides coverage for staff PTO and as needed with a minimum of 4 hours/month and/or up to 19 hours/week. The pharmacy is open 7:30am–4:00pm, Monday through Friday. After hours coverage is provided by TELEpharmacy. CPS Solutions, LLC has partnered with the hospital to manage the day to day operations of this pharmacy. This position is an employee of CPS Solutions, LLC. About the Pharmacy: Pharmacy hours are Monday-Friday from 7:30am - 4:00pm and utilizes TELEpharmacy remote services during off hours and weekends. The Pharmacy utilizes Pyxis and HCS. Required Qualifications: • Pharmacy degree (BS or PharmD) from an ACPE accredited school of pharmacy, PharmD preferred • Current Arizona Pharmacist license in good standing • Successful completion of a Pharmacy Practice Residency program strongly preferred • Experience as a Pharmacist in a hospital setting preferred • Ability to work independently, set priorities, make critical decisions, and respond to client concerns • Demonstrated accuracy and timeliness in a work setting • Experience using computer applications including word processing, spreadsheets, and pharmacy information systems • Effective communications skills, both verbal and written Hospital Requirements (may be required): • (PPD) TB Skin Test - Proof of negative TB skin test within the last 12 months • (MMR) Measles, Mumps and Rubella or A Blood Titer proving immunity • Varicella - (2) documented doses or A Blood Titer proving immunity • Hep B3 Series (or declination) • (Flu) Influenza shot - required for hire between Oct 1st - April 30th • COVID Vaccine - Full Benefits: All CPS Per Diem employees are eligible to participate in our 401(k) program with an employer match and the EAP (Employee Assistance Program).
