The Role:
We are looking for talented Pharmacists to join our team. Our Pharmacists deliver evidence-based pharmaceutical care services through direct interaction with patients and the healthcare team. This role encompasses patient and provider education, disease management, medication reviews, motivational interviewing, and ensuring the safe, cost-effective, and appropriate use of therapy. Who you are:
You are a patient focused certified/registered Pharmacist who is dedicated to delivering patient-centered care. You are passionate about helping patients learn how to better manage their medication regimens, and are excited to tackle our country’s suboptimal medication management problem.
Responsibilities:
• Delivering comprehensive medication management, chronic disease state management, and targeted medication reviews to assess appropriateness, safety, efficacy and adherence of all medications via virtual patient visits.
• Identifying barriers to adherence and optimize medication therapy to improve clinical outcomes through the delivery of evidence-based pharmaceutical services.
• Ensuring prescribing compliance with clinical practice guidelines, eliminating therapeutic duplication and delivering recommendations for appropriate medication therapy interventions
• Efficiently assessing prescribed medication therapy for unnecessary medications, need for additional medications, patient educational gaps, ineffective medications, inappropriate doses, adverse drug reactions, and adherence concerns
• Monitoring drug therapy and identifying areas for drug dosage adjustments based on therapeutic intent and patient's physiologic functions
• Identifying therapy interactions and contraindications
• Providing education to patients and the healthcare team regarding drug information, product availability, appropriate use and dosing, and adverse effects
• Generating patient-specific Medication Action Plans (MAP) through comprehensive medication reconciliation, written/verbal communication, feedback, and other outputs related to patient and provider interactions.
• Assisting with managing medication therapy affordability through cost-effective interventions which may include, but are not limited to, formulary or therapeutic interchanges, pill splitting opportunities, rebates and/or coupons available, combination products and elimination of unnecessary and/or duplicate medication therapies.
• Communicating, discussing and documenting clinical recommendations and/or interventions to prescribers and interdisciplinary care teams.
• Completing follow-up with healthcare providers regarding clinical recommendations.
• Coaching and training pharmacy interns, students and technicians as needed.
• Attending appropriate education programs in order to maintain licensure.
Essential Requirements:
• Doctor of Pharmacy from an accredited college or university
• Must be licensed in Arizona
Preferred Requirements:
• ASHP-accredited PGY-1 Pharmacy Practice Residency or at least 3 years of direct patient care experience
• Experience with Medicaid and/or Medicare programs
• American Pharmacist’s Association Medication Therapy Management Certificate Training Program certificate or equivalent
• Additional licensing in the following states is preferred: OR, CA, MN, AL
