Description
As an Initial Review Pharmacist, you will report to the Manager of Order Entry, monitor medication profiles, evaluate prescription drug orders, and make therapeutic recommendations to assure the appropriate and effective use of pharmaceuticals for patient care in a unique population.

Hours: Monday-Friday 2PM - 10PM CST

Responsibilities:
• Review and accurately verify prescription orders in a timely manner
• Assess, plan, intervene when necessary, and implement patient care
• Perform patient care activities to ensure safe and effective drug therapy
• Be of service to patients, physicians, nurses, and other pharmacists
• Articulate daily activities that support continuous quality improvement of patient care
• Maintain and improve job knowledge, skills, and competence
• Follows facility procedures when non-formulary drugs, target drugs, restricted drugs and investigational drugs are ordered
• Demonstrate proficiency in computer order entry and related computer functions
• Screen for drug interactions, allergies, order duplications, appropriate diagnosis, renal and liver function prior to order entry
• Assure facility queues are addressed in a timely manner
• Investigate, report, and document adverse drug events and medication incidents
• Document clinical interventions and follow-up when indicated
• Perform other duties as deemed necessary in accordance with established policies and requirements of accrediting bodies
• 24 Hour on-call responsibility 2-3 weeks per year
Requirements
Required Skills/Abilities:
• Doctor of Pharmacy (Pharm.D.) degree or equivalent experience with an active TX State Board Pharmacy license
• 3+ years of experience preferred
• Proven experience with medical and biological terminology
• Experience with FrameworkLTC and Docutrack software’s preferred
• Proficient with the use of Microsoft Office products
• Ability to function well in a high-paced and at times stressful environment
• Excellent attention to detail and the ability to balance competing priorities
• Must be able to be licensed in multiple states and have the desire to obtain multiple state licenses through the reciprocity process.
• Can de-escalate and troubleshoot problems within the pharmacy
• Can be available on short notice and/or when planned ahead
