Clinical Pharmacist - CT Licensed

A company is looking for a Clinical Pharmacist Market Support for the Clinical Excellence Team, working remotely. Key Responsibilities: • Conduct telephonic patient outreach and provider-facing activities to improve STARs measures • Analyze STARs and HEDIS reporting, providing insights and best practices to providers • Perform annual medication reviews and develop plans to enhance medication adherence Required Qualifications: • Doctor of Pharmacy degree or equivalent experience • Active, unrestricted pharmacy license in state of residence, with willingness to obtain additional state licenses • 5+ years of clinical pharmacy practice experience • 2+ years of experience with Medicare STARs and HEDIS measures • 2+ years of experience in provider-facing interactions and data analysis
